Once the season starts, if it becomes clear that the team has been placed in the wrong division, please fill out this form so we can make the necessary accommodations for the season.

In the spirit of fostering positive outcomes for students, we kindly request that you complete this form in the event that two schools are unable to resolve a conflict through direct communication. Together, we can work collaboratively towards a solution that benefits all parties involved.
